Joining the list

Please visit the surgery during normal working hours (08:30 – 18:00) and tell the receptionist you would like to register.  If you have your medical card, please bring it with you.  If you are registering your baby, please bring the form from the Registrar.  You are free to see any doctor you choose. You have the right to state a preference to receive care from a particular doctor or nurse. The receptionist will complete the registration form and make a screening appointment for you.

Leaving the list 

You can leave the practice list at any time. This may be because you are moving away but you may wish to move to one of the other local practices. You do not have to tell us you are leaving but it would be nice of you to let us know. In very rare circumstances you may be removed from the list.  This will only occur when we feel that the trust that must be present between you and the doctors no longer exists.
